Houston skydiving instructor dies after parachute fails to open

Skydive Houston, about 45 minutes outside the city, boasts on its website of offering a “safe, friendly and welcoming atmosphere for people to experience the thrill of human flight.” The skydiving equipment is the same tandem parachute system used by the US Special Forces.
